Michelle Poole, OTR  is an owner and serves as the Administrator and Executive Vice President for Greenfield Rehabilitation Agency, Inc.  She has been with GRAI since 1991.  Mrs. Poole is a Mount Mary College graduate with a Baccalaureate degree in Occupational Therapy.  She has over 21 years of experience and has dedicated her career to working with the geriatric and developmentally disabled client. 

Mrs. Poole has attended multiple continuing education opportunities with emphasis on PPS, Medicare Denial Remediation, Clinical Documentation, Restraint Reduction, and various clinical courses. 

Having been with GRAI her entire career, her main goal is to find the staff who ”fit” the company and what we stand for.  When hiring staff she not only finds qualified individuals, but people who have the same passion as we do – ensuring consistency in our staffing patterns.   She ensures that all aspects of the company continue to operate within the philosophies and traditions in which it was founded. 





Kate Brewer, PT, MBA, GCS, RAC-CT is an owner and President of Greenfield Rehabilitation Agency, Inc.  Mrs. Brewer has a Baccalaureate degree in Communication and a Master’s Degree in Physical Therapy from Marquette University as well has her Master’s Degree in Business Administration from Cardinal Stritch University.

 

Mrs. Brewer is a Certified Geriatric Clinical Specialist (GCS) by the American Board of Physical Therapy Specialties.  As one of only 14 Geriatric Clinical Specialists in the state of Wisconsin, this certification recognizes advanced clinical knowledge, experience and skills in the evaluation and management of the geriatric client.

 

Mrs. Brewer has been active in the Wisconsin Physical Therapy Association.  She has served on the state’s Practice Committee, Finance Committee, Board of Directors, Executive Committee, Delegation, District Chairperson and as the State Treasurer.  Mrs. Brewer is also active nationally and has served on the Finance Committee and Editorial Board for Gerinotes for the Section on Geriatrics and on the Nominating Committee, Government Affairs and Practice Committee for the Section on Health Policy and Administration.  Mrs. Brewer has also enjoyed serving on the Board of Directors for the Wisconsin Chapter for the American College of Healthcare Executives. 

 

Mrs. Brewer has several articles published and presented nationally on various topics including rehabilitation management, documentation and coding of therapy services and strategies for denial management.  She is the author of the book “The How To Manual for Rehab Appeals and Denials”, “Restorative Nursing Made Easy”, “The Essential Guide to Rehab Management” and “ICD-9 Coding for Therapy Professionals”.  She is a member of the editorial advisory board for “Briefings on Outpatient Rehab Reimbursement and Regulations, Outpatient Physical Therapy Coding Alert, and Eli’s Rehab Report. 

 

Mrs. Brewer manages the daily operations of Greenfield Rehabilitation Agency, Inc. and supervises the Regional Directors to ensure clinical and operational excellence at all of the facilities we serve.



Ginger Brath, OTR, MBA, RAC-CT:  I have been an OT since graduation from UW Milwaukee in 1992.  In 2008, after several long years of one class at a time, I earned an MBA from Concordia with an emphasis in Managerial Communications. It was worth the time, as I have enjoyed my various management roles and working with so many different groups of people. I have volunteered in many roles for the last 11 years in NBCOT, including item writing and validation of the OTR exam.  I have some specialty areas of Low Vision and Womens Health. I have been married 18 years to my high school sweetheart, Jim.  We have 3 children:  Alex 14, Mitchell 11 and Kailin 8.  We like to camp when we are not at a baseball game, spend time with our relatives and golf.  In the winter they do a lot of snowmobiling and sledding, but you wont see me outside any more than I have to.  I have enjoyed my transition to long term care and joining such a great group of people to work with!



Scott Leberg MS, CCC-SLP, RAC-CT:  Scott graduated with an undergraduate degree in communicative disorders and a graduate degree in Speech Language Pathology from the University of Wisconsin Stevens Point. In college Scott was a member of the Student Speech and hearing Association and the UW- Stevens Point football team.

 

Scott has been a member of Greenfield Rehabilitation Agency since June of 2005. He started with the company as a treating Speech Language Pathologist and Rehabilitation director. In July of 2007, Scott was promoted to the Regional Director position. He enjoys working for GRAI because of their strong ethical foundation and excellent clinical services that GRAI provides their customers.

 

Currently Scott is an active member of the American Speech and Hearing Association and has been recognized by this organization in the clinical competencies associated with speech language pathology. Scott has been certified in providing electrical stimulation (Vitalstim) to the muscles of the larynx to assist in dysphagia therapy. He also was certified, by  the American Association of Nurse Assessment Coordinators, as a Resident Assessment Coordinator in MDS 3.0.

 

Scott has a wife, Nicole, and three children (Elle, Nicholas and Chase). He enjoys spending his time coaching athletics and being active with his family. Some other interests include: golf, running, and weight training.



Becky Mueller, OTR/CLT, RAC-CT:   graduated from UWMadison Department of Kinesiology with Bachelors in Occupational Therapy. Additional area of focus included Assistive Technology education through the Wiseman Center.   

After graduation, Becky began her career with Greenfield Rehab Agency in May of 1995, specializing in SNF/Long Term Care and Geriatrics.   She began as a staff therapist, then advanced into the role of Occupational Therapy Manager and Scheduler,  held the position of Rehab Director for approximately 8 years, and currently functions as Regional Director for the Southern Wisconsin Region. 

 

She holds additional certification in Lymphedema Treatment,  Kinesio Tapeing, and MDS 3.0 Resident Assessment Coordinator through AANAC. Other areas of CEU focus include Modalities for OT,  Wound Care Management, Wheelchair Positioning, Craniosacral Therapy and Myofacial Release.

 

Outside of her career, Becky spends time with her husband and twin sons enjoying the outdoors all four seasons. She is a year round soccer mom and enjoys watching her 12 yr old boys snow board in the winter and skate board in the summer.  Biking the trails of Wisconsin and in her own backyard.  Camping, hiking and disc golf are a few other favorite family activities, along with the occasional quiet moment to read a book or relax with friends/and neighbors.
